Medication History Pharmacy Intern

Under the supervision of the Pharmacy Manager and under direct supervision of Pharmacists, this position is responsible for, but not limited, to providing specialized expertise in obtaining medication histories. The Medication History Pharmacy Intern shall obtain and document accurate medication histories for high acuity patients and all patients being admitted to the hospital as time allows, communicate to the clinical pharmacist any discrepancies found on medication histories that have already been reconciled by the admitting physicians to address any changes needed to inpatient orders, and expedite medications to the bedside by increasing coordination with central pharmacy. Obtain and document allergies, including allergy reaction, for all patients being admitted to the hospital as time allows. Obtain and document patient’s preferred outpatient pharmacy.
