Nurse Practitioner – HouseCalls (Contract Role)
Overview
The Client is seeking qualified Nurse Practitioners (NPs) to conduct in-home health assessments for Medicare Advantage members. This role focuses on preventive care, identifying gaps in care, and supporting improved health outcomes through comprehensive, patient-centered assessments.
This is a field-based, pay-per-assessment position with flexible scheduling across a 7-day work week.
Key Responsibilities
Clinical Duties
Perform in-home health assessments (45–90 minutes per visit), including:Medical history reviewReview of systemsPhysical examinationMedication reconciliationDepression screening
