Hours 8am to 5pm

Duties: Primary Care, Urgent Care.

Requirements:

IL license, BLS and DEA is required.

First 3 weeks will be 40 hours weekly. Subsequent weeks will be 8 hours every Wednesday and including the following dates: 11/23 and 12/28. Additional coverage could be added if provider available. Beginning mid to late Jan full-time M-F 40 hours weekly. Hours as below: Monday-8-5 Tuesday-10-7 Wednesday-8-5 Thursday-8-5 Friday-8-5
