NP opportunity to perform Annual Wellness Visits within the Northern part of Oregon

Great opportunity to work as a 1099 independent contractor for a well established group in Oregon. This Nurse Practitioner would be responsible for doing In-Home Annual Wellness Visits and Risk Assessments throughout selected counties.

• Board Certified Nurse Practitioner (NP)
• Counties to choose from in Northern Oregon (General Portland and surrounding areas) include:
  • Clackamas, Multnomah, and Washington counties
• Travel will consist of in home visits within a maximum of 55 miles
• Flexible work schedule during week to fit your needs
• Weekdays - Mon-Fri - 8a-5p - never on call
• Includes travelling time and visit duration - number of visits depends on the schedule you provide
• Paid per visit along with mileage - visits can take up to an hour
• Will see geriatric ages mainly
• Must have atleast 2+ yrs exp in primary care as NP, must have reliable vehicle, previous in-home risk assessment exp preferred
