JOB DESCRIPTION
OT @ Kenosha - CH Home OT
Coverage will be for primarily Racine and Kenosha Counties. Expectation of 6 points/ day

About Facility:
o Home Health position that covers Kenosha and Racine Counties in Wisconsin
General Information:
o Beds: N/A
o Minimum Years of Experience? 3 years minimum OT experience. SNF, home health, or acute care, but preferrable to home health experience
o Will you accept a first-time traveler? Yes
Patient Types/Common Diagnoses:
o All adults, strictly home health
o Geriatrics, post-stroke, post-joint replacement, weakness, deconditioning, COPD, CHF, etc.
o Will work with RN and PT interdisciplinary teams.
o Morning team huddle via phone every morning
Case Load:
o 6 patients/ points per day Evaluation visits are 1.5, standard visit is 1
