Occupational Therapist Home Health

Signature Healthcare at Home is seeking to add an Occupational Therapist for Snohomish County!
This position would cover Snohomish County (Everett, Marysville, Lynnwood, Edmonds, Bothell, Lake Stevens, Mukilteo, Arlington) for home health occupational therapy services.
This position works Monday through Friday day shift, with self-directed scheduling dependent on patient needs and territory. If you enjoy a high level of autonomy, travel and meeting patients where they are, apply today!
Duties and Responsibilities

• Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
  
• Provides patient, family and/or caregiver education, counseling, and refers patient to other home health services as appropriate.
