Job Description
3 Month Contract With A Local Authority Job Purpose To provide high-quality outreach and home-based Early Help support to children, young people, and families using a whole-family approach. The role focuses on delivering targeted interventions that improve outcomes, reduce risk, prevent escalation to statutory services, and promote the wellbeing and safeguarding of children and young people. Key Responsibilities Manage a caseload of children and families requiring targeted Early Help support. Complete Early Help Assessments (EHAT) and develop effective support plans. Act as Lead Professional, coordinating Team Around the Family (TAF) meetings and multi-agency interventions. Provide intensive outreach and home-based support to children, young people, and families with complex needs.
