The objective of the Outreach Program is to provide support to clients living with HIV, who are living with complex needs in relation to adherence to treatment and engagement in primary care services. The Outreach Program supports clients in Winnipeg through Nine Circles Community Health Centre and the Health Sciences Centre HIV Program site.

The Outreach Team works in tandem with the Social Work Team to support clients to be empowered in addressing the impact of the social determinants of health in their lives, and to have a voice in the various systems that affect them.

The job is a full time permanent position. The regular workdays for this position will fall between 8:00 a.m. and 8:00 p.m. Mondays through Fridays. Regular hours are 7.75 hours per day (net of breaks). Occasional evenings and wee...
