Pain Management Physician Assistant

Westmed Medical Group is currently seeking a full-time Weight Management Nurse Practitioner to work in collaboration with a physician on our Weight Management team primarily at the Purchase, New York location. Primary responsibilities include delivering high-touch, high-value assessment and care management services for comprehensive weight management in a multi-specialty group practice. Practice includes in office patient engagement, which allows for comprehensive patient centered care. Patient care and assessment in collaboration with the PCP and Multi-specialty care team including full medical history, medication review, Weight, Height, Waist circumference, Total cholesterol / LDL / HDL / triglycerides, Fasting glucose, Insulin, Hba1c, Systolic and diastolic blood pressure as well as behavioral and psychosocial assessment and use of data analytics to capture a comprehensive picture of the patient's health status and appropriate care planning and motivate change for desired outcom...
