Patient Enrollment Representative

This role is responsible for enrolling patients into Chronic Care Management (CCM) programs, ensuring regulatory compliance, and collaborating with healthcare providers for a smooth enrollment process.

Position: Patient Enrollment

Shift timings: 7:00 pm - 4:00 am

Location: Malam, Lahore (on-site)

Key Responsibilities

• Manage the CCM patient enrollment process
• Explain program details and benefits to patients
• Coordinate with providers/NPs for patient eligibility and documentation
• Track patient progress and ensure timely follow-ups
• Collaboration with sales team

Required Skills and Experience

• Experience in healthcare or patient enrollment
• Knowledge of CCM programs and regulations
• Strong attention to detail and excellent communication skills
• Proficiency with healthcare management software
