Patient Support Specialist

The Patient Outreach Specialist serves as a liaison between healthcare providers and patients, aiming to enhance patient engagement, improve health outcomes, and ensure continuity of care. This role involves contacting patients to schedule appointments, providing education, following up on care plans, assisting with care coordination, and supporting medication prior authorization workflows.

Key Responsibilities:

• Conduct outbound calls to patients for appointment reminders, follow-ups, preventive care outreach, and care gap closure.
• Educate patients on available health services, wellness programs, treatment plans, and medication adherence.
• Assist in coordinating care between departments, including scheduling tests, specialist referrals, and other healthcare services.
