Essential Duties and Responsibilities:
Providing exceptional door to door care to all PACE Participants & Centra PatientsFor PACE: Entering safely into participant’s homes on a routine basisCommunicating all participant/patient changes & needs to direct supervisorPerforming pre & post vehicle safety inspections, assuring vehicle is in proper operational condition and reporting any maintenance issues to immediate supervisorMaintain cleanliness of interior & exterior of Centra vehicleFollowing scheduled transports for PACE participants or patients & other potential trips communicated by direct supervisorFor PACE: Delivery of participant medications, medical supplies and other medical necessitiesMaintain all necessary records/documentation regarding participants/patients & vehiclesCommunicating frequent changes to direct supervisor Other Functions:
