As part of the multi-disciplinary team with the National University Health System (NUHS)@Home, you will provide quality pharmaceutical care as our Pharmacist .

• Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions.
• Medication counselling on transfer medications by a variety of means (in-person and virtual).
• Review discharge prescriptions appropriately.
• Verify new orders by the physicians and identify inappropriate medication/doses if relevant.
• Assist with performing medication supply duty.
• Follow up on medication preparation status.
• Undertake duties which may be assigned by the NUHS staff member depending on the requirements.
