Description

Brigham and Women’s Anticoagulation Management Service (AMS) is a pharmacist-run clinic that provides full anticoagulation management services for all oral anticoagulants including warfarin (Coumadin, Jantoven), dabigatran (Pradaxa), rivaroxaban (Xarelto), apixaban (Eliquis), and edoxaban (Savaysa). Brigham & Women’s (BWH) AMS has been recognized as a Center of Excellence by the Anticoagulation Forum. Patients are referred to our service by a Brigham and Women’s/Faulkner Hospital attending physician, usually the patient’s primary care provider or a specialist such as a cardiologist or hematologist for management of anticoagulation therapy. BWH AMS is a service dedicated to caring for patients who require anticoagulation.
