Job Description
**BAYADA Home Health Care** is seeking an experienced **Physical Therapist, PT** , for an opportunity performing home health visits for our Denver office.
This is a **Weekend Physical Therapist** position - as visits will include mostly Start of Care / Admissions, Medicare Certified Home Health and OASIS documentation experience is required.
Per Diem, Part Time and Full Time scheduling options, including:
+ **$5,000 Incentive if able to commit to 4 SOC per weekend (2 Saturday + 2 Sunday)** **OR**
+ **$5,000 Incentive if able to commit to 20 productivity points per weekend (combination of Fri, Sat, Sun, and/or Mon) - benefits eligible!**
As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
