Team 3: Ashroken, Centerport, Cold Spring Harbor, Commack , Dix Hills, Eaton's Neck, Elwood, Fort Saonga, Greenlawn, Half Hollow Hills, Hauppauge, Huntington, Huntington Bay, Huntington Station, Kings Park, Lloyd Harbor, Melville, Northport, Northport East, & Smithtown.
Fee-for-Service Physical Therapists are required to:

+ Work a minimum of 2 shifts per pay period (every 2 weeks)

+ Attend the full first week of orientation, Monday through Friday from 8:30 AM – 4:30 PM

+ If no prior CHHA experience, commit to training 2...
