Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona PT license or multistate license valid for Arizona is required to start the assignment.
• BLS Certification from AHA or ARC may be required.
• Experience providing 1:1 outpatient physical therapy care preferred. New graduates open for general outpatient caseload.
• Experience with EMR systems preferred; client uses Prompt.
• Pelvic floor physical therapy experience required for a portion of the caseload (approximately 20 hours); candidate must have prior pelvic PT exper...
