**Physical Therapy Assistant-Rehab-Per Diem-Day-Mount Sinai Queens**

Provides treatment according to a plan developed by and under the supervision of a licensed physical therapist, maintains communication with the physical therapist regarding patient plan of care, documents appropriately and maintains their own statistics.

**Qualifications**

+ **Education:** Associates Degree
+ **License/Certification:** Physical Therapy Assistant (PTA) Issuing Agency: NYS/Department of State
