Responsibilities

• Conduct Patient Assessments and Triage


• Educate on health, chronic health symptoms (diabetes, hypertension, etc.)


• Follow up with clients/members with their Doctor's recommendation of care


• Coach lifestyle changes to clients/members to improve overall health


• Link to the nearest healthcare provider
