Shift - 8:00 AM-4:30 PM

Pay - $30/hr - $35/hr



PURPOSE OF POSITION

Quality Management Coordinator will support the Quality Management Team through the offboarding process of external care management agencies. This will involve the quality review and evaluation of care coordination (CC) records and completion of CC chart audit activities, tracking of corrective action, and support the processing of case status changes in the Electronic Medical Record (EMR) for our external agency partners.

SUMMARY OF DUTIES AND RESPONSIBILITIES

1. Provide support to the Quality Management team by attending offboarding meetings and managing the offboarding tracking grid.


2. Chart Audits: Assess the quality and Product of care coordination services and documentation in the Epic electronic medical record system using the Health Home s chart audit tool for the offboarding process and beyond. Ma...
