Summary:
The Referral Coordinator assists patients with scheduling follow-on care as referred by a care provider.
Responsibilities:
• Pulls referrals from fax or electronic work cue and logs appropriately for tracking purposes. Forwards referral to physician for review and approval. If referral is not approved, prepares and mails/faxes notification letter to referring provider.
• Once referral has been approved, contacts patient to conduct medical history interview and schedules first appointment. Initiates new patient checklist. Enters patient information into electronic medical record. Verifies insurance information for referral purposes as well as patient’s mailing address. Assists patient with special needs requests such as transportation, interpreter needs, et...
