Holland Home of Grand Rapids is seeking a full-time Dietitian to join our amazing team of health care professionals on our beautiful Breton Woods Rehabilitation & Living Centre here in Grand Rapids! Our Skilled Nursing and Assisted Living facilities are committed to delivering the highest quality care to the older adults we are honored to serve.

Responsibilities:

Reviews physicians' diagnoses and designs therapeutic nutritional care plans.
Monitors and documents patient progress, consulting with physicians as needed.
Educates staff, residents, and families on diet plans and performs dietary counseling.
Ensures food service operations comply with nutritional plans.
Integrates and implements nutritional needs and diet restrictions with other departments.
Reviews and documents nutritional care plans to improve residents' eating functions.
