Registered Nurse - Care Coordination and Integrated Case Management (CC&ICM)

Summary The RN CC&ICM Initiative Coordinator is administratively responsible for the clinical programming and daily operations of the Clement J. Zablocki VA Medical Center (CVAMC) CC&ICM initiative. The RN (CC&ICM) Initiative Coordinator has both clinical and administrative responsibility to ensure effectiveness of services, application of latest VHA guidelines and modification of service patterns. Responsibilities The primary purpose of the CCICM RN is to provide coordination and direct clinical evidence based services including: Assessments and interventions Program development Veteran, family, caregiver and staff education on Care Coordination (CC) and Integrated Case Management (ICM) Resource development Community outreach Continuous quality improvement activities Evaluation/consultation The CCICM Coordinator is administratively responsible, in collaboration with the CCICM Social Worker (SW), for the clinical programming and daily operations of the CCICM throughout the facility.
