Position Summary:

The Referral and Scheduling RN is essential to the seamless coordination of referrals and scheduling across Home and Community Services, supporting a broad range of programs including hospice, home care, palliative care, and other specialized community services. This role manages the referral through admission, and scheduling processes ensuring patient visits are scheduled appropriately to maintain manageable caseloads and high-quality care.
