Responsibilities:
- Perform ongoing patient assessments and document clinical findings.
- Administer medications and treatments according to established protocols.
- Notify the MD/NP of pertinent patient information and changes in condition.
- Coordinate patient admissions, discharges, and transitions of care.
- Provide ADL assistance and deliver individualized nursing support.
- Manage specialized care needs, including G-tube care, urinary catheter care, nebulized treatments, wound vac care, CPAP care, and tracheostomy care.
