Registered Nurse - RCI- Referral Coordination Initiative

Summary The VA Northeast Ohio Healthcare System is recruiting for a Registered Nurse - Referral Coordination. The Registered Nurse - Referral Coordination will function in the Referral Coordination Initiative Service (RCI) section of the Medical Center. Responsibilities Duties of the Registered Nurse - Referral Coordination include but are not limited to: Communicates with Veterans on their health care options including local VA, VA Medical Center, and community-based eligibility. Serves as a liaison between referring providers and specialty care services. Works closely with other VA Medical Centers, contracted facilities, and community facilities to remove obstacles that would impede the Veteran from utilizing the services at other VA medical centers. Outpatient specialty clinic care coordination and patient flow management, including appointment management.
