Skills: 6 Months to 2 Years of direct clinical experience required (acute care, ambulatory, home health, or care management)

We are hiring a Care Coordinator (LPN/RN) with the following skillset:
Key Responsibilities
• Provide telephonic and virtual patient assessments, care coordination, and ongoing
monitoring for assigned patient populations.
• Deliver patient and family education related to conditions, treatment plans, medications,
and self-management strategies.
• Develop, implement, and update individualized care plans based on clinical guidelines
and patient needs.
• Monitor patient progress, escalate clinical concerns appropriately, and support early
identification of deterioration or barriers to care.
