Evaluates assigned patients, gathering data from a variety of sources, such as patient records, interviews, observation, and team members. In accordance with patient's assessment and physician’s prescriptions, develops treatment plans for assigned patients including specific goals and -evaluates and modifies plan at periodic intervals and as necessary.Discusses treatment plans and patient progress with physician and appropriate hospital staff; recommends alterations to the treatment plan when considered advantageous to therapy purpose.Exercises judgment in the delegation of activities to ensure that patient care is provided by staff with identified competencies in meeting individual patient and family needs.
