Purpose of the Role : The Dialysis Practitioner delivers safe, evidence-based and person-centred dialysis care, placing the patient's goals, preferences and outcomes at the centre of clinical decision-making.  Working collaboratively within the multidisciplinary team, the practitioner promotes effective care coordination, shared decision-making and patient self-management to improve clinical outcomes, patient safety and the overall patient experience.
