Job Description
Join Baycrest Hospital as a Registered Practical Nurse (RPN) in the Complex Continuing Care Unit. Deliver patient-centered care and collaborate within an interdisciplinary team to meet diverse needs.
This permanent part-time position involves working day-evening shifts with alternate weekends. You will care for older adults with complex medical conditions, focusing on improving their quality of life and addressing holistic care strategies. Engage with families to support individualized care plans and enhance patient well-being.
Key Responsibilities:
• Plan and evaluate nursing interventions and medications
• Deliver client-centered care with an interdisciplinary team
• Collaborate on individualized care plans with Registered Nurses
Requirements:
• Current CNO registration in good standing
• Basic Cardiac Life Support Level C certification required
• Preferred experience with geriatrics and complex needs
• Excellent interpersonal and communication skills
