Job Description
Enhance community mental health services with Ontario Shores as a Registered Practical Nurse in Whitby, Ontario. This full-time role focuses on supporting individuals with dual diagnoses and their transitions.
In this permanent role, you will apply your clinical nursing skills within the Dual Diagnosis Transitional Housing Rehabilitation Program. Working closely with multidisciplinary teams, you will be key in developing behavioral plans and facilitating community education to assist in psychiatric transitions and track client progress.
Key Responsibilities:
• Collaborate with inpatient teams for timely referrals
• Develop and supervise behavioral management plans efficiently
