RN CASE MANAGER - POSITION HIGHLIGHTS

Location: Visalia, CA Hybrid - Remote
This is a hybrid, field based role requiring a dedicated home office and the ability to travel 3-4 days per week within an assigned territory. Coverage includes the Visalia, CA and surrounding areas. Territory assignments may change cased on business needs.

Schedule: Monday - Friday
Flexibility is required to accommodate patient needs and nephrology partner availability.
