JourneyCare Hospice is seeking an RN Case Manager to join their growing team in the Riverwoods area. This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes. Now offering up to $97,000 annually! Offer based on years of experience.

Hours: Monday-Friday 8:30am-5pm and one weekend every 12th week (4 weekends/year)

Territory: Riverwoods, IL 60015

Qualifications:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid driver's license and proof of insurance is required
