**Job Summary and Responsibilities**

As our RN Health Coach you will help patients, physicians, advanced practice providers, staff, and other professionals coordinate patient care across the continuum so they can achieve improved health outcomes and navigate the healthcare system effectively. Every day you will manage disease registries by identifying and following up with patients not meeting clinical goals based on evidenced-based guidelines, and serve as a liaison with specialty practices, in-patient facilities, and community resources. You will be expected to work autonomously, use education, goal setting, action planning, problem-solving, and motivational interviewing to support health behavioral change, and contribute leadership to the healthcare team.
