We are seeking an **Adult Congenital Nurse Coordinator** to join our Eastowne Cardiology Clinic team. This role offers the opportunity to coordinate patient care and serve as a key resource within a dynamic outpatient cardiology practice.

Candidates with previous cardiology and/or ICU experience are strongly preferred.

Schedule and Location:

+ Hybrid Work Schedule, Monday-Friday, 8a-5p
+ No weekends
+ No holidays

Summary:
Provides clinical administrative nursing care coordination in support of a patient care area. Duties include one or more of the following care coordination, discharge planning, data analysis and performance metrics, and patient/staff education.
