Client Health Connex enables the secure electronic exchange of patient health records among healthcare providers, improving care coordination and public health outcomes. The Care Management Analyst will support the HIE Medicaid Services (HMS) program, a collaboration between Client HIEA and the Client Department of Health and Human Services (NCDHHS) Division of Health Benefits (DHB), also known as Client Medicaid. The program focuses on improving data exchange to enhance Medicaid quality and population health efforts through: Transitioning to digital quality measures (dQMs). Improving health-related social needs (HRSN) data exchange. Enhancing care management data sharing.
