About the Role
The Senior Social Worker collaborates with the multi-disciplinary team to ensure the delivery of excellent, person centred care for consumers and their families, carers, supporters. The role provides leadership and works to build capacity and capability within the social work team, focusing on psychosocial care, discharge coordination, and safeguarding.

Direct consumer care

• Comprehensive psychosocial assessment, Risk Assessment, Discharge planning and family work.

Leadership and capacity building
