What will your responsibilities be?
In this role, you will support individuals aged 18+, handle transitional cases and handover work with children services on a case-by-case basis. You will work with adults in the community, hospital, rehabilitation, and care settings within Calderdale, as well as support individuals placed in care settings outside of Calderdale.
