Social Worker - Adults - Learning Disability (Gr )

To support young persons transitioning from children (age 17 years old) to adults services using a number of legislations such as the Children & Family Act 2014 & Care Act 2014; working collaboratively with key stakeholders such as children services, adult services, education and health colleagues as well as directly with our clients in a person centred/strength based way. Minimum of 3 years post qualified (social work) experience - is necessary for this post.
