Social Worker (Community Mental Health Department, Central Region)

Our Community Resource, Engagement and Support (CREST) teams support clients who are at risk of or diagnosed with mental illness as well as their caregivers. The teams provide basic emotional support and monitoring of their mental health conditions through home visits and service linkages.

• Engage and collaborate with stakeholders such as government agencies, grassroots, and community-based organizations to raise awareness of mental health
• Conduct mental health screening to identify those who are at‑risk/diagnosed with mental health conditions and connect them with the right services for early diagnosis and treatment
• Provide basic emotional and cognitive support to clients with mental health issues and/or their caregivers and link them up with relevant services based on their needs
