Job Title: Social Worker – Community Mental Health Team – Rochdale

Liquid Personnel is currently recruiting for a Social Worker to join a Community Mental Health Team based in Rochdale.

What will your responsibilities be?
Undertake statutory adult social care functions within a secondary mental health service, working with individuals experiencing severe and enduring mental ill-health.

Complete strengths-based assessments, scheduled and unscheduled reviews, care planning, and commissioning of support in line with individual needs.

Carry out Mental Capacity Assessments, Section 42 safeguarding enquiries, and facilitate timely hospital discharges in collaboration with health colleagues.
