Social Worker – Early Supported Discharge (Home Rehab)

Health New Zealand is seeking a passionate and experienced Social Worker to join their Early Supported Discharge service in Palmerston North. This permanent part-time role focuses on supporting patients transitioning from hospital to home, working with multidisciplinary teams and whānau.
