            Navigator Homes of Marthas Vineyard - Looking for a Full Time Licensed Social Worker for Navigator Homes

JOB SUMMARY

Assists in meeting the psychosocial needs of residents in a long-term care facility.  Collaborates with the inter-disciplinary team to develop and carry out individualized care plans to enable each resident to achieve their optimal level of health and well-being and maximize their quality of life.

QUALIFICTIONS

MA LCSW required.
