Social Worker - Emergency Department

Job Summary:

In this position, the Social Worker will provide comprehensive psycho-social assessment, management and support to at risk patients and families at Upstate Medical University Hospital. This position provides coverage to the entire downtown campus during evening hours with weekend rotation coverage to support throughput in the Emergency Department and inpatient units during high volume times. The Social Worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual psychosocial needs. Candidate must possess strong written and verbal communication skills, have knowledge of available community resources, social determinants, population health and drivers of utilization.
