Trinity Health Of New England is looking for a Licensed Social Worker to join our Cancer Center Team. 

The Social Worker LCSW will support our Cancer Center located in Enfield, CT. The LCSW will assess and assists in addressing and meeting the psycho-social needs of cancer patients and families, enabling patients to utilize medical care and other supportive services to achieve their optimum level of health and function.

You Will Be Responsible For: 

• Assesses and counsels’ cancer patients/families referred to the Cancer Center.
• Provides consultation to physicians and staff in ambulatory discharge planning, from treatment, survivorship services, to psychosocial distress assessment.
