Liquid Personnel is currently recruiting for a Social Worker to join an Adults LD team based in Stockton on Tees.

What will your responsibilities be?
In this role, you will support adults with learning disabilities, including young people transitioning into Adult Social Care. You will conduct Care Act (2014) assessments, develop person-centred support plans, and review care packages. Applying the Mental Capacity Act (2005), you will conduct capacity assessments and Court of Protection work. You will collaborate with health colleagues to support those detained under the Mental Health Act (1983) and assess eligibility for Continuing Health Care (CHC) funding.
