Position Summary
The Advanced Heart Failure Program is seeking a dedicated and experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to provide comprehensive care to patients with complex heart failure and those undergoing evaluation for advanced therapies, including heart transplantation. The APP will work collaboratively within a multidisciplinary team to deliver exceptional patient care in the outpatient setting.

Key Responsibilities
- Provide comprehensive assessment and management of patients with advanced heart failure, including medication optimization and adjustment of guideline-directed medical therapy (GDMT), diuretics, and other therapies.
- Order, interpret, and manage diagnostic studies, including laboratory tests, echocardiograms, and other cardiac imaging.
