Job Description
Patient Care
- Participates in the planning, delivery and evaluation of patient care within the haemodialysis clinical setting:
- Undertakes clinical/patient assessments pre, during and post dialysis treatment to evaluate effectiveness of care
- Adheres to all FMC Policies and Work Instructions related to clinical and operation management
- Reviews blood test results and reports to Medical Director any abnormalities
- Educates patients on their diet, fluid, medications and blood pressure management to optimise their clinical outcomes
- Motivates and encourages patient’s adjustment to CHD
- Monitors the water treatment system and fluid for dialysis to ensure provision of safe, quality dialysis therapies
- Establishes an effective relationship with patients and maintains good communication with patients and relatives; and gives adequate support.
