Serve as the primary case manager for the ASSIST Team (Behavioral Intervention Team).

Conduct non-clinical assessments of student needs, identifying barriers to success including personal, social, and situational challenges.

Develop individualized action plans that support students’ educational/personal goals and persistence.

Provide ongoing case management, including regular follow-ups, progress monitoring, and adjustment of support strategies.

Maintain accurate, timely, and confidential case documentation.
