Supervisory Registered Nurse

SYNERGY HomeCare Delray Beach is a CHAP-accredited non-medical Home Health Agency serving clients in southern and central Palm Beach county. We have been in business for 16 years. We are currently seeking a per-diem part-time Supervisory RN to join our team.

As a Supervisory Registered Nurse with SYNERGY HomeCare, you will:

• Conduct comprehensive home health patient assessment during Client Start of Care Admission and assessments on an ongoing basis in the clients’ homes. “Admission” includes completion of an agreement with the patient or the patient's legal representative to provide home health services. Implement plans of care to achieve positive outcomes.

• Assist in handling Client emergencies, as well as client grievances and complaints.

• Comply with applicable law and regulation.
